How to complete a Police Check

1. Gather Personal Information:
   • Collect necessary personal details (e.g., previous addresses, license numbers).
2. Access the Police Check Service:
   • Click here to access the Self Service Police Checks For CityLife Church Inc.
3. Understand the Terms:
   • Select the check type: "Volunteer/Student - Organisation Funded."
   • Review information about CrimCheck Ltd and their role.
   • Acknowledge electronic submission risks and consent to personal information collection.
   • Read and accept the Terms and Conditions.
4. Complete the Online Form:
   • Enter personal information accurately (full name, birth details, contact info).
   • Provide current and previous residential addresses for the past five years.
   • Specify the purpose ("Volunteer") and the requesting organization ("CityLife Church"), and location ("Victoria").
   • Write the ministry you are volunteering in (e.g., Kids, Youth, Weekend Services).
   • Upload necessary identity documents for verification (up to 9 documents, max 16MB per file).
5. Submit Your Application:
   • Review all information carefully.
   • Submit the application within 30 minutes to avoid session timeout.
   • As a volunteer, this service is free of charge. You do not have to pay anything.
6. Await Confirmation:
   • Receive an email confirming your submission (check junk email folder if not received).
